Application:
Congestive heart failure, pulmonary edema, hypertensive crisis, edema-ascites syndrome in liver cirrhosis, renal failure, nephrotic syndrome, marked hypernatremia, hypercalcemia, and hypermagnesemia.
Contraindications:
Hypersensitivity (including to sulfonamides), hepatic coma, severe electrolyte derangement, hypokalemia, marked deficiency of the liver and kidneys, oliguria for more than 24 h, anuria, gout, hyperuricemia, diabetes, or breach of tolerance to carbohydrates, decompensated mitral or aortic stenosis, increased pressure in the jugular vein, more than 10 mm Hg. Hypertrophic cardiomyopathy with obstruction of the left ventricle output tract, hypotension, myocardial infarction, systemic lupus erythematosus, pancreatitis, metabolic alkalosis.
Side effects:
Hypotension, including postural, collapse, thromboembolism, thrombophlebitis (mostly elderly), hypokalemia, hypomagnesemia, hyponatremia, impaired glucose tolerance, hyperuricemia, gout,
Overdose:
Symptoms: hypotension, reduction of ORC, hypokalemia.
Treatment: vital functions keeping.
Dosage may be increased by 20 - 40 mg every 6 - 8 hours (large dose divided into 2 - 3 hours). V / m or / - single, 20 - 40 mg (if needed - increasing the dose of 20 mg every 2 h). Jet w / introduction produced slowly within 1 - 2 min. In high doses (80 - 240 mg or more) given intravenously in drip at a rate no higher than 4 mg / min. The maximum daily dose - 600 mg.
